Obituary of Sammy Lee Middlebrook

Mr. Sammy Lee Middlebrook ,  72 , of Polk City , Florida  passed away January 5, 2022  at  home  surrounded by his loving family . He was born on May 27, 1949  in Tampa , Florida  to Louis  and Jeanette  (Allen ) Middlebrook .  Sammy was a former Director of Meat and Seafood division for Publix Super Markets where he retired after 47 years of service.  Sammy proudly served his country as a veteran of the U.S. Air Force.  He loved raising his cattle, riding his tractors, traveling in his R.V., boating, and he loved spending time with his family whom he loved very much.  He was a faithful member of the Green Pond Baptist Church of Polk City, Florida.

Sammy  is survived by: his loving wife of 27 years, Wanda; 3 sons, Jason Middlebrook of Winter Haven, Florida, Jeff Middlebrook of Lowell, Indiana, and Jon Middlebrook of Winter Haven, Florida.  He also leaves to cherish his memory, 9 grandchildren and 3 great grandchildren.

The family will receive friends at Green Pond Baptist Church located at 5995 Green Pond Church Rd, Polk City, Fl  on Monday January 10, 2022  from 10AM  -  11AM.  Memorial services will follow at 11AM.
